Western Australian recycling outfit Recycla-Plas was fined $50,000 last week over an incident that led to the amputation of two of a 15 year old worker's toes.
The incident had occurred in 2006 while the worker was assisting the operation of a baling machine when his foot was crushed between one of its rams and a box located nearby.
"Anyone in control of a workplace containing machinery with hazardous moving parts needs to ensure that those moving parts are safely guarded," WorkSafe WA's Commissioner Nina Lyhne said.
A $1,700 modification could have prevented the injury, she added.
