Hazards need to be redefined for incident investigations
The definition of “hazard” in the safety industry is too broad, and as a result, investigators, risk or OHS managers struggle to not only identify a hazard in an incident investigation, but to clearly identify which risk control measure may have either failed or been omitted, according to an expert in hazards and related investigations.
