Before you begin filling out your application please ensure you have read and understand the information about the application process and the fees, criteria and requirements associated with each certification type. 

We also recommend you ensure you have the required documentation available before commencing the application. This includes:

  1. A current CV/Resume
  2. A transcript of your program/s of study (Please note we require a copy of your transcript which shows your subjects undertaken)
  3. Your Demonstrated Capability Assessment Report - one of three outlined below (PDF only):
  • If you are applying for Generalist OHS Practitioner Certification you must complete a practitioner portfolio report  - Download the template.
  • If you are applying for Generalist OHS Professional Certification you must complete a professional practice report - Download the template.
  • If you are applying for Chartered Generalist OHS Professional Certification you must complete a reflective journal - Download the template.
